LifePerks for Health provides you with 24 hour a day, 7 days a week telephone access to Board Certified Physicians. These doctors can consult, diagnose and prescribe medications over the phone and call in any prescriptions to your local pharmacy immediately. With your LifePerks for Health Prescription Card you will pay only $2.50 above wholesale pricing. With LifePerks for Health, you also receive deep savings on Medical Diagnostics, Imaging, Blood Work, Lab Work and more. LifePerks for Health also provides deeply reduced pricing for all Dental treatment, Vision (glasses and exams), Hearing Aids, Prescription Drugs.
As a LIfePerks for Health member you will enjoy additional access by voice mail, email and smart phone 24/7 to Board Approved Licensed Physician, Dentists, Physical Therapists, Personal Trainers and other professionals. LifePerks for Health gives you unlimited access to over 500,000 medical providers, over 40,000 Alternative healthcare providers and over 33,000 Pharmacies throughout the US. Plus, LifePerks for Health can be used by your entire family for their on-going treatment needs without having to reapply.
Note: LifePerks for Health Medical Concierge have what is termed as an “open network”. In other words, you have the choice of all local physicians and medical facilities, not being limited to a certain network of providers.
2. How does LifePerks for Health work?
Simply fill out a short application online or call one of enrollment professionals at 1-800-643-6108.
Once you get started you can use your LifePerks for Health Dental your other benefits go into effect within 30 business days. Your HealthPerx Plus card and packet will arrive within three weeks. Just show your card and receive your savings immediately. If you need additional assistance call your LifePerks for Health Patient Care Coordinator at 1-800-643-6108. They will help you make the proper arrangements to assure you pay only the reduced fee.
3. How can LifePerks for Health help me?
LifePerks for Health will reduce your medical costs by giving you access to quality doctors 24/7. Dental costs by as much as 80%, and all dental procedures are covered. On Vision you will save an average of 10% to 50% on glasses, lenses and frames. For you and your family’s prescription drug needs you will pay only $2.50 above wholesale pricing. Additionally you will save up to 58% off on Hearing Aids and FREE Audiology Consultation.
4. How can I find a location of providers near me?
Within your members packet you will receive a providers list that applies specifically to your area. Simply select a dentist or vision provider, make an appointment and show your card at the time of service. Just click on the link “Provider Search” and select a benefit. This will take you to a site that will help you find a provider nearest to you.
5. How can I find out which doctors accept LifePerks for Health?
LifePerks for Health you will not have to change your physician. When you need additional care our Concierge will help you find an Physician, Specialist or Medical Treatment Facility in your local area based upon Patient Satisfaction and at Reduced Affordable Rates. For Your Dental and Vision needs you will receive a packet within a few weeks that contains a detailed instructions to help you find all professionals and facilities in the area in which you live. Our Concierge Specialists can walk you through the process giving you information on the providers that according to our customers provide the highest level of satisfaction. For your Dental, Vision and Prescriptions Drugs benefit simply show your card at the point of service. Calling ahead the facility will not usually give you information regarding pricing. Pharmacy Benefit is available at over 33,000 pharmacies nationwide.
6. How can I qualify for LifePerks for Health?
There are no pre-existing requirements for our plans. You will be accepted regardless of age or pre-existing health conditions. You will enjoy no limits, no pre-qualification, no paperwork and no additional costs.
7. Can I apply for LifePerks for Health online?
Absolutely! Applying online is quick and easy. Simply complete and submit your application to enroll. Call your Patient Care Coordinator to arrange for any testing or treatments you may need. Your LifePerks for Health packet and card should arrive within a few weeks. It’s that easy! Click “Apply” under each product description and enroll on the secure and confidential form.
8. What would my monthly payments be?
Click on “Products” on the Menu Bar to see your available options. LifePerks for Health is as little as $11 a month for the individual and just $55 for our most popular program that covers the entire family. LifePerks for Health is affordable to all families and provides benefits for the entire household. Call or ask one of our Patient Care Coordinators and they will help you select a program that best suits your needs and budget.
Your rates are Guaranteed Renewable for life will not increase no matter how much you save or how many times you use your plan.
9. How do I pay for my plan?
It’s easy. Payments are set up as Automatic Bank Draft, Credit Card, Payroll Deduction or a Semi-Annual or Annual Billing.
10. Does my LifePerks for Health plan have a limit?
That is another beauty of LifePerks for Health. Your benefits are unlimited, regardless of the number of times you use it, your age or how much you save. Unlike other plans that have annual limits, you don’t have to wait another year to get the treatment you need. Remember LifePerks for Health covers all pre-existing conditions.
Household means any person who is dependent upon you financially. Yes, this can be your children up to age 27 who reside at home or are enrolled in a college or University. If you have parents or other loved ones who are financially dependent on you they will also qualify for coverage under your family plan.
